Applicant Guideline

Prior to receiving your Live Scan Fingerprinting with us, you must obtain a Request For Live Scan Service (BCII 8016) form (except childcare licensing applicants) from the requesting employer or agency. Our Live Scan Fingerprinting Centers accepts Requests for Live Scan Service Forms from any requesting employer or agency approved for Live Scan by the DOJ (provided the forms are complete).

It’s the applicant’s responsibility to obtain the appropriate forms. Please be aware that we will be unable to provide any Live Scan Fingerprinting service unless you have the necessary request form and proper identification.

What You Will Need – Appointments Requested

Obtain A “Request For Live Scan Service” Form
- At least one (1) “Request for Live Scan Service” form issued to you by the requesting agency, employer or download from the internet. To expedite your Live Scan process, please ensure form is completed prior to arrival. As a courtesy, you can find some of the forms on our website under “FORMS & LINKS”.
An Acceptable Form of Identification
- California Driver’s License
- Out-of-State Driver’s License
- Department of Motor Vehicle ID Card (Commonly known as DMV for most states.)
- Passport
- Alien Registration / Immigration Green Card
- Military ID Card
Payment – We are set up to accept the following forms of payment

- Major Credit Cards – Visa, MasterCard, AMEX, & Discovery
- Check or Money Order
- Cash (Exact amount requested.)
