There are many agencies that require applicants to use the FBI Applicant Fingerprint Card Form FD-258 in order to conduct a FBI Identification Record—often referred to as criminal history record and/or FBI criminal background checks; also know as a “rap sheet”. Basically it is a listing of certain information taken from fingerprint submissions retained by the FBI in connection with arrests and, in some instances, federal employment, naturalization, or military service. All arrest data included in a FBI Criminal Background check is obtained from fingerprint submissions, disposition reports, and other information submitted by agencies having criminal justice responsibilities.
Print Legibly, Use Black Ink
All data entered on ink fingerprint cards must be typewritten or legibly printed, preferably utilizing black over blue ink and must not exceed the boundaries of the designated field (block).

FBI Applicant Fingerprint Card Form FD-258 are submitted when an applicants apply for non-law enforcement position and require FBI Criminal Background Checks completed as part of hiring and/or licensing processes (i.e. nursing professionals, teachers, day care providers, security guards, physicians, school bus drivers, racing commissions, liquor licenses, etc.).
Will My Fingerprint Card Be Returned?
No. Due to concerns related to the protection of personally identifiable information, the FBI Applicant Fingerprint Card Form FD-258 will no longer be returned either for a “no record” response or with a FBI Identification Record. Certified & Trained Technicians
When selecting a fingerprinting location be sure to that the individual taking the prints are highly trained and/or certified by a legitimate organization such as Live Scan Fingerprinting Centers (CAL-LIVE) or your local law enforcement agency. One of the fingerprint roller technician’s main responsibility is to seek good quality of prints. When completing the FBI Applicant Fingerprint Card Form FD-258 each individual fingerprint impression is to taken to roll the fingers from nail to nail; and to obtain simultaneous plain “flat” impressions at 45 degree angles that do not extend up into the rolled impressions.
